Improving Physician Documentation Specificity
Increasing physician knowledge and proficiency with Advocate Health Care's clinical information systems.
- Anatomic site (e.g., elbow, trunk, shoulder, etc.) and laterality (i.e., right or left)
- Degree of the burn (i.e., first, second, or third)
- Burn type (e.g., thermal, radiation, electric, corrosive [by chemicals], etc.) and substance causing the injury (e.g., fire, gasoline, bleach, hot toaster oven, hot water, etc.)
- Extent of body surface involved based on the rule of nines (e.g., 10%, 20-29% … 80-89%, etc.)
- Place where injury occurred (e.g., apartment, kitchen, orphanage, bathroom, prison cell, etc.)
- Intent of injury (e.g., accidental, intentional, assault, etc.)
